The above placed in one envelope clearly marked with your
SUMMATIVE ASSESSMENT NUMBER and 'VIDEO
FOR SUMMATIVE ASSESSMENT ONLY' and delivered by hand or sent
Royal Mail Special Delivery to your Deanery Office
MRCGP/SA Single route video

Summative Assessment Number and MRCGP examination number, clearly
labelled on the Workbook, videotape, and on the videotape box in
accordance with the instructions issued by the RCGP

2 copies of Video Assessment of Consulting Workbook (original plus copy) issued by RCGP and completed correctly in accordance with RCGP Instructions

COGPED Video Declaration Form completed and signed
The above placed in one envelope clearly marked with your
SUMMATIVE ASSESSMENT NUMBER and 'VIDEO
FOR MRCGP/SA SINGLE ROUTE' and delivered by hand or sent
Royal Mail Special Delivery to your Deanery Office
It is the GP Registrar's responsibility to ensure that the submission
has been made in accordance with the RCGP requirements.
Should the videotape fail the MRCGP video module, it will be
'fast-tracked' and automatically enter the local deanery summative
assessment process.
