Requirements of GP Registrars

  1. Present the Audit
  1. Under the headings as set out on COGPED 8 criteria audit proforma, which should be highlighted in their word processed/typed submission.
  2. Double spaced
  3. All pages numbered
  4. Summative Assessment number in the top right hand corner of each page.
  5. Bound in a way that ensures all pages are firmly fixed in order
  6. No longer than 3,000 words - this may include tables, glossary and references
  7. Accompanied by the completed and signed COGPED Audit Declaration Form.
  1. The Audit must be anonymised which means that the GPR's name, their GP trainer and/or GP practice's name does not appear on the Audit before submission. Deanery Administrators will under no circumstances be able to delete names from audits. If an Audit is not anonymised, it will be returned to the GP registrar, which may result in a significant delay in certification.
     
  2. Submit
  1. 3 copies of the COGPED Audit
  2. Completed and signed COGPED Audit Declaration Form

to the Deanery Office via 1st class mail no later than 3 months before the end of their general practice based training year. (There is no need to send these via Royal Mail Special Delivery.) The copying of the audits is expected to be provided at the expense of the GP training practice.

  1. Keep a copy of the Audit for their own reference in the event of a request for resubmission.
